Family History Book

For ease of completion, you can give us your information by filling in the information form using Adobe Acrobat.

1. Download Adobe Acrobat, if you don't have it.(click the GET ADOBE ACROBAT button below). 
2. Once downloaded, click either the McCrary History Form button or Grove History Form button below to get the corresponding information form. 
3. Save the form to your computer.
4. In Adobe, open the file, click the Tools/Edit tab, select Type Text (the icon is a dotted box with an A inside)
5. Move to each section on the form, click and insert your information.
6. Save your form. Email it to the appropriate person OR print and mail it as directed.
7. Remember to also email a current photo.
